All address and name changes must be submitted to the Board office in writing or via email. All name changes must include a photocopy of the marriage certificate, divorce decree, etc. There is a $10.00 fee for ordering a new license certificate. Submit fee, written request, and appropriate certificate of name change to the Board office.
