It’s time to renew your OT license! All OT licenses expire June 30, 2010 if not renewed. A completed 2010/2012 Renewal of Licensure form, renewal fee, and continued competency verification must be postmarked on or before June 1, 2010 to avoid the additional $100 late fee.

Continued Competency: You must submit verification of 20 hours of continued competency with your renewal form and fee. Continued competency is valid if it was earned within 24 months prior to the date your 2010/2012 renewal application is received by the Board office. For example, if your renewal is received in the office on May 23, 2010 and your continued competency is from April 21, 2008 and May 29, 2008. The May 29th course would count because it is within 24 months prior to May 23, 2010. The April 21st course would not count since it is not within 24 months of that date.
Also you cannot submit a course more than once. If you used any course for the last renewal, you cannot use it for this renewal. Hours do not carry over from one renewal period to another. However, the late fee still goes into effect if everything (renewal form, fee, and 20 hours of CE) is not postmarked on or before June 1st.
ATTENTION ALL NEW GRADS!: According to the Board's continued competency criteria, those who have graduated with a degree in occupational therapy during the licensing period (July 1, 2008 to June 30, 2010) may use their academic coursework as continued competency for renewal. Submit a copy of your transcript with the renewal application.
If you were initially licensed from 7-01-2009 to 12-31-2009, you will only need 10 contact hours. If you were licensed before 7-01-2009, you need to submit 20 hours of continued competency verification. Verification must include a certificate of completion or a statement of your attendance signed by your supervisor.
The 2010/2012 renewal period is for two years ending June 30, 2012.

must be submitted to the Board office in writing or via email. All name changes
must include a photocopy of the marriage certificate, divorce
decree, etc. There is a
$10.00 fee
for ordering a new license certificate. Submit fee, written request,
and appropriate certificate of name change to the Board office.
