OT/OTA NOTICE! Please read  Licensure Requirements  to ensure timely processing of your license Application for State Licensure Substantiation of Supervision (OTA) Self Assessment Verification of Licensure (if applicable) Limited Permit OT/OTA Applicants for a limited permit must take the examination within four months of completion of the education and experience requirements. Therefore, limited permits may only be issued to those who have graduated within the past four months! NOTICE! Please read the  Limited Permit Requirements  ensure timely processing of your license. Application for State Licensure  Limited Permit Substantiation of Supervision  Self Assessment  Late Renewal of License 2010 Renewal of License If you wish to reinstate your ND license and you have not been licensed for less than 3 years, you must submit the 2010 Renewal of License form, submit the renewal fee plus the late fee, and submit 20 hours of continued competency earned within the 24 months prior to the completed application for renewal of license.
